According to AP News, the cause of death cited on Ozzy Osbourne's death certificate is a heart attack. Osbourne also suffered from coronary artery disease in addition to Parkinson's diease, which also contributed to his passing. The Black Sabbath frontman and reality TV star died at the age of 76 on July 22.
